1. Must have significant hearing loss in one or both ears
2. Must be a resident of the state of Illinois (citizenship not a requirement)
3. Currently a high school senior in good standing
4. Financial need will be considered
5. You will need 2 letters of recommendation to complete your application (can be submitted on the form below or mailed in to the address in the application)
6. Applications are due May 3, 2024
